POLICIES AND PROCEDURES
The following is a listing and explanation of the policies and procedures used by Manna Counseling Service. They are provided for the client as a means of informing them regarding certain procedural practices so that misunderstanding can be avoided. I. Appointments
- All clients must read and sign the Duty to Warn form before engaging in any formalized therapy.
- Cancellation of a previously scheduled appointment except for emergencies must be made within 24 hours of the appointment time. Failure to adhere to this policy may result in the client being charged for the session. Missed appointments will result in the client being charged for the session.
- Appointments are regularly scheduled on weekdays from 9:00 am. to 5:00 pm. A limited number of evening appointments are available. A ten dollar premium will be added to these appointments.
- Sessions are fifty minutes in length. Since the fifty minute period begins at the scheduled time, it is in the client’s best interest to arrive on time.
II. Fees
- Fees are assessed based on income level and other life circumstances.
- Fee reduction is at the discretion of Manna Counseling Service.
- Unless otherwise arranged, the client agrees to pay at the time of each session.
III. Billing
- Billing statements are mailed near the end of each calendar month. Any incurred balance is due upon receipt of the statement.
- Failure to remit payment on time may result in interest penalties being added to the account. Balances that are overdue more than 30 days will be charged interest for late payment. Every effort will be made to work with clients to resolve their accounts, however, Manna Counseling Service reserves the right to utilize the collection services of a third party agency for balances more than 60 days past due.
- If your insurance company covers all or part of pastoral counseling services, you may submit the statement you receive from Manna Counseling Service to your insurance provider after you have paid your account directly to Manna. Insurance company reimbursement is the sole responsibility of the client.
- A $25.00 charge will be added to the client’s account for retuned checks.
IV. Other
- All business should be conducted during regular business hours of 9:00 am. to 5:00 pm. Monday through Friday.
- All reasonable effort is made to protect the client’s confidentiality. Signing a Waiver of Confidentiality form, engaging in behavior that is dangerous to self or others, illegal activity and defaulting on one’s account are ways that the client relinquishes confidentiality.
- The client agrees to pay for replacement of any loaned materials that are list or damaged during the course of treatment.
- By engaging counseling services with Manna Counseling Service, the client agrees to adhere to all of the above policies.
